HOW TO APPLY FOR CHANGE MATRICULATION?

Steps to apply for change matriculation:

  1. Secure a Change Matriculation Form at the Office of the University Registrar (Form No. 9);
  2. Go to the concerned college and department that offers the subject/s you wish to change for approval;
  3. Let the assigned evaluator cancel the desired subject and enter a new one;
  4. Pay the Change Matriculation fee at the University Business Office (UBO);
  5. Go to the Office of the University Registrar for validation;
  6. Proceed to Window 3 for printing of the new COR reflecting your new schedule and subject.

HOW TO APPLY FOR DROPPING OF COURSE/S?

Steps to apply for Dropping of Courses:

  1. Dropping of courses starts after the Prelim Examination has been held and it lasts only for a specific period of time. Please refer to the university academic calendar for the schedule;
  2. Secure a Dropping Form (Form No. 5) at the Office of the University Registrar;
  3. Fill out the dropping form;
  4. Accomplish the form and obtain the signatures of each teacher who handles the subject you are currently enrolled in but you wish to drop;
  5. Obtain the signature of your academic adviser and the Dean of your college;
  6. Pay for the dropping fee at the University Business Office (UBO); and
  7. Submit the form together with the receipt at the Office of the University Registrar’s Office for recording.
